This is a student work refusal form. This makes tracking student work refusal easy! It has different sections where the student fills out. Including what the assignment is, why they are refusing to complete the work, as well as a place for students to sign the form. This not only teaches personal responsibility, but also accountability for students!

Do you have a student who isn't motivated to work and refuses to do anything? You should try the assignment refusal slip! Most of the time, just setting the slip down and asking them to fill it out motivates them to start working on their original assignment. If it doesn't, you have a paper trail showing parents, administrators, and SPED instructors. If they refuse to fill out the form, add that to the notes and fill out the name section for your records.
