I have a small bathroom but also two small kids. Luckily there's the possibility of hacking a perfect diaper changing solutions for small bathrooms.

This is a change table we made to fit above our toilet. The mat is held in place by adhesive backed velcro (that way it doesn't interfere with the hinge when it's closed. We tested it with a 35 pound weight plate before putting our 8 pound daughter on it! I don't think we'll use it once our kid is over 20 pounds (if we are still changing diapers). It's great having a change table right next to our sink.
