Describe the bug

When adding a file in a folder after starting copilot in the same folder, the file can't be found with @. Only files show up that existed before.

Steps to reproduce the behavior

1. create folder with 2 file a.md b.md
2. start copilot in that folder
3. add another file c.md
4. try to reference @c

Expected behavior

c.md should show up.
